Clinical Effectiveness of Family Constellation - Evidence map

Family and Systemic Constellation is a therapeutic approach developed in the 1980s aimed at understanding and resolving relational dynamics, particularly within the family context. This map presents an overview of the evidence regarding this practice for various health outcomes. Based on a comprehensive literature review, 16 articles published between 2009 and 2024 were included, comprising 13 studies with primary data and 3 reviews. Main Findings ● The studies evaluated the effects of Family Constellation across 3 groups of interventions: Family Constellation Seminar, Family Constellation Session, and Family Constellation Workshop. ● The interventions were associated with 12 health outcomes distributed across 2 groups: Psychological and Behavioral Indicators, and Pathological Conditions. ● A total of 18 associations between interventions and outcomes were identified, with the majority (11) involving Family Constellation Sessions and outcomes in the Psychological and Behavioral Indicators group. ● The group of Psychological and Behavioral Indicators outcomes received the majority of the associations (15), followed by the Pathological Conditions group (3). Among the outcomes, notable outcomes were Psycho-emotional Behavior Change (4), Psychological Well-being (3), and Psychological Domain (2). ● The other 9 outcomes were each associated once. ● A positive effect for 1 association was reported for the outcome Psychological Domain. ● Potentially positive effects were reported for 16 associations, mainly for the outcomes: Psycho-emotional Behavior Change (4) and Psychological Well-being (3). ● An inconclusive effect was reported for Psychopathologies, and no negative effects were reported. Implications for Practice and Research The findings suggest that Family Constellation may offer benefits mainly for mental health, reinforcing its potential as a complementary therapeutic approach. However, the low reliability of the available evidence highlights the need for more robust studies to confirm these effects and better understand its mechanisms of action. For clinical practice, the approach may be considered as a complementary strategy but should be applied with caution and integrated into other evidence-based therapeutic methods. For research, it is recommended to conduct well-designed clinical trials, with larger samples and rigorous methodologies, in order to create an evidence base on the safety and efficacy of Family Constellation for health outcomes.
